The FY25 Chapter 257 attestation survey is now live on MassGRANTS.
Providers are encouraged to provide feedback but are not required by EOHHS. The FY’26 survey will be released to providers towards the end of the year.
To complete the survey, please log in to the MassGRANTS portal. Providers should have been sent walk-through instructions on how to complete the survey should they have any questions.
This survey will close by 11:59 p.m. on June 30 due to licensing agreements.
