Awards of Excellence

There are many individuals and organizations that deserve to be honored for their creativity, contributions, and accomplishments within the human services community, setting the standards of excellence in our community-based sector.

In recognition of their contributions, the Providers' Council honors deserving individuals and organizations each year with our Awards of Excellence program at our annual convention. All honorees are nominated by peer Council member agencies for these awards.

We hope you will join us at our 41st Annual Convention & Expo on October 24, 2016 to honor our awardees and to celebrate their contributions to the sector.

To nominate an individual or an organization for an award, you can click here to use our online form or you can download a hard-copy PDF that you can fill out and send to us.

The deadline for Awards of Excellence nominations is June 17, 2016.
