Early Intervention Program
EI Physical Therapist
The Eliot Early Intervenition Program of Eliot Community Human Services in Malden is seeking a Physical Thereapist. This provides a muliti disciplinary supports to children under three and their parents.
Responsibilities:
Identifies, screens, and evaluates performance of children using standard measures.
Provides intervention in the areas of postural control, mobility, muscle tone, range of motion, and needs for adaptive devices and prostheses.
Provides transdisciplinary services to families and children enrolled in the program.
Develops and ensures follow up to treatment goals in the Individual Family Service Plan (IFSP). Plans are developed in accordance with the requirements of the funding source, sound clinical practice, and in collaboration with the family.
Utilizes a team approach to treatment planning and service delivery with other staff members, family members, physicians, and agencies.
Generates assessment reports, IFSP's, progress notes, and insurance verification reports within established timelines.
Provides inservice training to other team members within areas of expertise.
Qualifications
Bachelor's Degree in Physical Therapy. Registered in Massachusetts by the Board of Registration in Allied Health. Meets Department of Public Health and third party regulatory requirements. Must have good clinical documentation skills and be able to meet deadlines.
Bilingual Spanish, Portuguese or Haitian Creole preferred
