Position Description

Seeking clinically sophisticated, and responsible adults in the Holyoke/Chicopee area.  Qualified host will teach individual independent living skills, provide safe and stable living environment, have reliable transportation, and be able to work as part of a team and set limits when necessary.
Responsibilities:
• Provide in-home support/ assistance with scheduling, attending appointments, budgeting and money management.
• Supervise/facilitate community involvement and leisure activities
• Complete required documentation, attend team meetings and trainings as needed
• Help with ADL skills.

Qualifications

Minimum Requirements:
• BA or minimum three years relevant experience.
