Position Description

CHD’s Adult Mental Health program is seeking a full-time, 36-hour per week, RN position within the CBFS system that coordinates functions and activities that provide for, maintain, and promote the health of clients in need of Department of Mental Health funded services.   Works closely with program staff and medical providers to ensure timely, appropriate care for participants in DMH-funded Community Based Flexible Support program.  Provides trainings to participants and staff regarding medication function, effects, side effects, nutrition, specific disease processes which apply to program clients, and vital sign monitoring.  Accompanies individuals to medication reviews and medical appointments as necessary.  Attends planning and support meetings as necessary.  Maintains detailed records in accordance with agency and funding source standards, including the use of an electronic health record.  Two years experience working with psychiatric population and registration with the Massachusetts Board of Nursing.  Ability to work independently and as part of a professional team in a fast-paced environment.  Excellent communication skills are necessary.  Send resume and salary requirements  by July 17, 2012 to Jeffrey Trant, Program Director, 622 State Street, Springfield, MA  01109 or to Jtrant@chd.org.  CHD is an AA/EEO Employer.  Minority candidates are encouraged to apply.
