Position Description

Conduct psycho-social assessments and develop and implement community care plans for elders participating in a coordinated care health insurance program.  Work with interdisciplinary team, including primary care physician and RN.  Bachelor’s degree and at least six months elder care experience required.  35 hour work week.  Fluency in Portuguese a plus. This position offers a comprehensive benefits package and requires a valid driver’s license and reliable transportation.   EOE/AA
