CLINICAL COORDINATOR WALTHAM
Coordinate and provide individual, group and family therapy as needed, and coordinate treatment team meetings to discuss Individual Service Plans, progress reviews, re-entry plans and discharge plans.
Oversee the medication, medical needs, and mental health of residents.
Oversee advocacy and phase components of the program, and Oversee, monitor, facilitate and provide feedback on effectiveness of daily groups.
Coordinate, monitor and facilitate special groups (substance abuse, violence prevention, DBT, victim awareness, etc.)
Assess new referrals and the intake and discharge process/planning working closely with the department, and arrange or conduct testing and evaluations as needed including CANS, and assessments.
Work closely with the Program Director regarding clinical issues, and Organize and ensure all staff receives required clinical training..
Maintain and submit written reports that are complete, accurate and timely as required by funding sources, licensing agencies and the program/agency. Ensure that required information is entered into the Virtual Gateway according to policy.
Serve as on-site administrator as scheduled and be included in the administrative on-call and overnight check rotation.
Qualifications
Master's degree or higher in a related field required. Must be licensed as an independent practitioner, or be license eligible. At least three years experience with adolescents and children. DBT certified or willing to participate in certification program. Knowledge and experience with clinical issues of adolescents. Ability to work a flexible work schedule contingent upon program needs, and ability to work effectively in a fast environment with periods of high stress.
Must be at least 21 years old and posses a valid driver's license and reliable transportation to and from work and for the transportation of clients to and from appointments, family visits, school, and activities as needed. Must have reliable telephone notification system for emergency contact.
