Recognize Your Staff With an Awards of Excellence Nomination

There are many individuals and organizations who deserve to be honored for their creativity, contributions and accomplishments within the private provider industry. These individuals and organizations set standards of excellence.

We invite you to nominate individuals within your agency and community organizations that you work with for this important honor.  Help us recognize outstanding contributions within our sector with an Award of Excellence nomination.

• Awards will honor individuals for performance between June 1, 2008 & June 1, 2009.
• Want a list of all the awards for which you can nominate someone? Or just want to see what types of awards we present?
• Only member agency employees are eligible for the private provider awards.
• Any provider vendor is eligible for the Business Partnership Award.
• Prefer working with hard copy? Download the Nomination form and fax (617-428-1533) or mailed to the Providers’ Council. Also, be sure to download the Nominee Accomplishments form and send that back as well so we can learn more about your nominee.
• Deadline for all nominations is July 10, 2009.

Recipients will be honored at the Annual Convention and Expo, October 29, 2009.
